Goitsimang Mosemane (38) and Anastacia Ramapa (40) were shocked when they went to their gogo's grave on Friday and found that part of it had been dug up.
Their grandmother's head cloth was lying outside the grave in Kagiso Cemetery, west of Johannesburg.
The grandmother was buried two years ago.
Goitsimang told Daily Sun: "We don't think our gogo's skull is in the grave because her head cloth and pieces of the coffin were outside.
"We have applied for an exhumation to find out whether or not her skull is still there.
"This is really stressful because we will have to bury our gogo for a second time after the exhumation."
The hole in the ground was made directly where their grandmother's head was.
Because the soil has caved in over it, it is not possible to see whether her skull is still in the coffin or not.
Goitsimang added: "We suspect this is the work of Satanists because they obviously didn't want to steal the coffin and there was no tombstone over our gogo's grave."
Goitsimang and Anastacia said they received a call from an undertaker on Friday telling them to rush to the grave.
Anastacia said: "The undertaker said that he was alerted of the incident by the cemetery caretakers."
Police spokesman Warrant Officer Solly Sibiya said: "The matter was reported and a forensic team was sent to the scene. We are waiting for the results and cannot confirm whether the skull is missing or not."
